Families and Community Together (FACT) is the Union County Care Management Organization (CMO).  We work with Union County youth (ages 5 up until their 21st birthday) and their families who may be experiencing challenges with mental health, behaviors, substance use, and developmental disabilities.  FACT partners with youth, families, and their communities to develop plans that will help the youth achieve stability, positive change, and success at home, school, and in their community.
